For questions regarding insurance or claims, please contact:Claims Reporting 

• Automobile:  Auto claims will be reported to Waldorf Risk Solutions either on their website, https://waldorfrisksolutions.com/customer-service/, via email to [email protected], or via phone at 631-423-9500. If emailing the claim, please use the "Auto Claim Form" here
• Property: Property claims will be reported to Gallagher Bassett either online via: https://expedited.gbgofirstconnect.com/springfield-property, by email to [email protected], or by phone 833-226-3221. If emailing the claim, please use the "Property Claim Form" here
• General Liability:Liability claims will be reported to Gallagher Bassett either online via:  https://expedited.gbgofirstconnect.com/springfield-liability, by email to [email protected] or by phone 833-226-3221. If emailing the claim, please use the "General Liability Claim Form" here.
• Worker’s Compensation: Worker’s Comp claims can be reported to Constitution State Services by email at: LossRptCSS@constitutionstateservices.com or by Phone at: 1.800.243.2490. Our contract number is: TCSSC 305T7987, account number: EB3. 

Clergy-Most vehicle registration renewals can now be processed online. If your vehicle registration requires an insurance stamp for the Massachusetts Registry of Motor Vehicles to process your renewal application, please mail your vehicle registration renewal form to:Before mailing-make sure to sign the back of your vehicle registration renewal form, enter your license number and include a check made payable to:  MassDOT for the amount of your registration renewal. If you do not provide all of the aforementioned items, your registration renewal cannot be processed. Please send a copy of all vehicle registrations, each time they are renewed, to [email protected]. Failure to do so could result in a lapse in coverage and/or registration. Please note that the Finance office does not have the insurance stamp and any documents requiring the stamp must go through Mary Ann.

Certificates of Insurance: To request a Certificate of Insurance for General Liability or Property, you must fill out the form below and email it to [email protected]. It is helpful if you can also send the expired/expiring Certificate of Insurance for reference.
Requests for a Workers Compensation Certificate of Insurance should be emailed to Amanda Taillon at [email protected].

Special Event Insurance – If Parishes/Schools rent out their property, whether for meetings or events, renters must have Special Event Insurance. This includes receptions held after funerals and all non-parish sponsored group meetings. Special Event Insurance is a protection for the Parish and the Diocese in case of accidents. Please email a completed Special Event Insurance Request Form and a Facilities Usage Indemnity Agreement to a.patry@diospringfield.org to purchase Special Event Insurance.
